DUTIES:
The Housing and Urban Development (HUD) Veterans Affairs Supportive Housing (VASH) program is a collaborative effort between the U.S Department of Housing and Urban Development and the Department of Veterans Affairs. This program combines HUD Section 8 rental assistance vouchers with the VA Social Worker's profession of intensive case management services in order to move Veterans and their families out of chronic homelessness and into permanent independent housing. A primary component of the program is VA case management services, which are designed to improve the Veteran's physical and mental health and to enhance their ability to remain stable, housed and community-integrated. The incumbent, a professional social worker, independently functions as one of the social worker case managers for the VASH program. He/she provides the case management services necessary to ensure that Veteran care is coordinated with the VA medical center, community health or mental health providers and other agencies that serve homeless populations either directly or through referral.

Duties include:
Development, coordination and delivery of high quality and intensive Social Work Services.

Providing clinical supervision, intensive case management and macro-level organizational services at an advance practice level.

Interaction with Veterans, family members, significant others, community members and other staff in a positive manner.

Conducting and participating in outreach activities including field interviews, assessments and referrals for homeless Veterans.

Referral, screening and admission procedures for potential HUD/VASH Veteran participants that meet national policy and medical center criteria.

Develops psychosocial treatment plans in coordination with interdisciplinary team members, based on psychosocial assessments, including goals for clinical treatment of HUD/VASH eligible Veteran participants.

Other duties as assigned.

This position requires a current valid Drivers License and the incumbent may be required to transport Veterans eligible for the HUD/VASH program.

Work Schedule: This position may require a flexible work schedule.

Relocation expenses and/or incentives have not been authorized.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:

Physical standards - All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.

License: Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level and must be able to provide supervision for subordinate staff to qualify for licensure.

Education: Individuals must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.

At least two years should be in an area of specialized social work practice.

Veterans with service-connected disability of less than 10%, nonservice-connected disability, and Purple Heart recipients, as-well-as those claiming derived preference for a spouse, widow or widower, or natural mother, should refer to and submit a completed SF 15, "Application for 10-Point Veteran Preference," and required documentation with your application materials.
