This position is a "hybrid" that will serve in the capacity of Crisis Clinician at the St. Johnsbury office and in the capacity of "Team Leader" at the St. Johnsbury Care Bed. Under assigned supervision of the Emergency Services Coordinator, the Crisis Clinician provides a variety of clinical services to mental health and/or substance abuse clients, including crisis intervention, clinical case management services, consultation and education services to area schools, agencies and the community at large. This position is responsible for the collection and oversight of dispositions, follow-up referral verifications and interfacing with other NKHS departments in order to better serve client needs when in crisis.

DUTIES

DUTIES & RESPONSIBILITIES - CARE BED TEAM LEADER

Essential Functions:
The primary job function is to provide for the physical, emotional and adaptive needs of the consumers, including and not limited to, medical care, physical care, training assistance in daily living skills, care and supervision of all activities of daily living, under the direct supervision of the Director of Emergency Services.

Duties and Responsibilities:

  • Provide for a safe and healthy living environment for housed consumers.
  • Provide direct care to consumers to meet their physical needs. This includes, but is not limited to, bathing, toileting, lifting, transferring, and physical assistance.
  • Under direct supervision of the Emergency Services Director, operate the Care Bed within grant guidelines.
  • Provides day-to-day supervision to Care Bed staff.
  • In concert with the Emergency Services Director, coordinate and facilitate team process at the Care Bed.
  • Participates in interviews of prospective staff.
  • Assist staff with the completion of all required documentation and paperwork and conform with all regulations that apply to the provision of services and NKHS policy.
  • In the absence of the Emergency Services Director, consults with Division Directors to ensure the Care Bed program continues to provide appropriate and necessary services to clients and the community.
  • Participate in program development and implementation.
  • Assume primary responsibility for screening referrals to the program during normal Care Bed work hours.
  • Responsible for scheduling Care Bed staff.
  • Attend Treatment Team and staff meetings as assigned.
  • On an "as-needed" basis, may assist in crisis clinician function during scheduled Care Bed hours.
  • Demonstrate an ability to follow Behavior Protocols.
  • Must successfully complete Medication Delegation and Special Care Procedure training and maintain skills in order to safely provide direct care as prescribed by Physician or Licensed Professional.
  • Comply with state, federal and agency regulations.
  • Implement and participate in the Individual Support Agreement.
  • Complete documentation according to state, federal, and Agency criteria during assigned shift.
  • Complete household tasks of cleaning, laundry, meal preparation, etc. Maintain residential and work areas in a clean and safe manner.
  • Document health and safety issues to Director of Emergency Services.
  • Assist consumers in relating to family, community, and other team members in a competent and respectful manner.
  • Provide family support to ensure success of consumers.
  • Properly care for and maintain agency transportation.
  • Maintain strict consumer confidentiality in accordance with Federal Confidentiality Regulations (42 CFR Part 2 as amended) and NKHS policies.
  • Must be a team player, able to work flexibly with the needs of consumers, and the needs of the agency and co-workers.
  • Must participate as requested in supervision and attend staff meetings.
  • Must demonstrate good organizational skills and judgment.
  • Must complete 12 hours of continuing education training per year.
  • Utilize computers and available technology within the NKHS system, i.e., charting assessment results, reports, progress notes, and time sheets in a timely manner as identified by the supervisor, in accordance with state, federal, and program regulations and guidelines.
  • Report any instance of consumer abuse to their direct supervisor according to NKHS policies, and the appropriate authorities, according to state laws.

  • Covers after hours emergency services as deemed appropriate.
  • Coordinates services within NKHS and with other agencies and providers as needed.
  • May provide specialized assessment/screening services based on professional concentration and training.
  • Consultation and Education Services to community at large, area agencies, the professional community and/or schools as required. Consultation can be educational or evaluative in nature. Specific area of consultation/education are based on clinician's areas of expertise or interest. Provides liaison services between two sponsoring agencies so as to coordinate services to clients.
  • Clinical Case Management and Social Support on an as needed basis. Services may include service coordination with other agencies and services; collateral contacts with other family members or significant others in client's life; telephone support to clients or collaterals; social support services for location, developing, or providing care services; or treatment planning session.
  • Coordinates with client's psychiatrist (if applicable) and other medical providers, both within and outside agency.
  • Clinical Record-Keeping. Initiates and maintains in a timely and thorough manner the wide variety of record keeping and administrative documentation required to account for our service to clients. Such work includes, but is not limited to, client-specific assessment, case services, and progress notes, time sheets, and schedules.
  • Coordinates with client's insurance carrier for service authorization/utilization review purposes, providing information required for such purposes within agency confidentiality guidelines.
  • Provides documentation as needed in legal proceedings and appears in court as required.
  • Clinical Team Meetings. Attends and actively participates in the necessary clinical meetings designed to inform, problem solve, and coordinate services and program.
  • Documentation, including but not limited to, all emergency contacts, writing treatment plans, progress notes, daysheets, inactivity/closing reports, correspondence, periodic reports and any other required information.
  • Develop positive working relationships with NKHS staff, community service providers and individuals/families served.
  • Participate in training pertaining to job skills, duties and responsibilities.
  • Assist with developing and maintaining emergency service protocols.
  • Other duties as assigned by supervisor that may be necessary to meet the needs of individuals/couples/families served.
QUALIFICATIONS

Requires a minimum of a Bachelor's degree in a recognized field of mental health that includes or is supplemented by 12 college credits in social work, psychology, social welfare or other relevant social sciences (including credits in areas of mental health or child welfare, as appropriate to position responsibilities) (1-2 years experience) and at least one year's experience dealing with acute psychiatric assessment and dispositions.

Experience in provision of public or private social work, clinical services, mental health, developmental services and/or substance abuse preferred.

Must possess the ability to effectively interface with a wide range of medical and professional clinicians in hospital/clinic settings and be detail focused for the purposes of accurate and timely documentation and adherence to Federal, State, and Agency standards.
